Overview of services provided

The Wellness Recovery Team (WRT) works collaboratively to improve physical and behavioral health issues in those diagnosed with serious and persistent mental illness, substance use disorders, and physical health conditions. The Administrative Navigator, Behavioral Health Navigator, and the Nurse Navigator jointly assess and support individuals by identifying areas of concern and work together with all of the individual’s health providers. This includes, but is not limited to, primary care providers, managed care organizations, specialists, psychiatrists, therapists, as well as family and other supports. All services are delivered within the context of a strong commitment to recovery-oriented care and recovery-oriented practices. Services are individualized and strengths based.

The WRT works with the individual to:

  • Improve understanding of medication
  • Improve self-management and wellness strategies, including, but not limited to, stress management, diet/nutrition, exercise, and use of preventative care
  • Develop improved relationship skills
  • Increase confidence in relating to physicians of all types, and understand how to negotiate and communicate with all health care providers
  • Increase coordination of care
  • Increase advocacy for one’s own health

Where are services provided?

Services are provided where the individual is at the time of need and may include their home, doctor’s office, community settings, or a Merakey office.

Who is eligible for services?

  • Individuals must be at least 18 years old
  • Be diagnosed with a severe and persistent mental illness (SPMI) and a chronic physical health condition
  • Montgomery County: Have Medicaid benefits through Magellan Behavioral Health of Montgomery County
  • Lehigh County: Have Medicaid benefits through Magellan Behavioral Health of Lehigh and Northampton Counties OR be eligible to receive mental health services through Northampton County Human Services

Who provides WRT services?

The team consists of:

  • an Administrative Navigator,
  • a Master’s Level Behavioral Health Navigator, and
  • a Registered Nurse Navigator.

This team works together with the identified individual to provide quality services focused on whole health and wellness.
